Is Compounded Semaglutide Safe? What the FDA Warnings Mean Before You Choose a Provider

Compounded GLP-1 pricing can look attractive, but the safety question is specific: what exactly is being compounded, by whom, under what legal basis, and why is it clinically appropriate for you?

Summary

FDA says unapproved GLP-1 products are not reviewed for safety, effectiveness, or quality. FDA has warned about fraudulent compounded semaglutide and tirzepatide products with false labeling. The shortage context changed after tirzepatide and semaglutide injection shortages were resolved, narrowing routine compounding pathways.

FDA-approved and compounded are not the same category

FDA-approved drugs have approved labeling, manufacturing standards, and postmarket safety systems. Compounded medications can be appropriate in specific patient situations, but they are not FDA-approved and are not evaluated by FDA for safety, effectiveness, or quality before use.

Red flags before buying compounded GLP-1s

  • The site will not name the pharmacy.
  • The prescriber relationship is unclear.
  • The medication is described as generic Ozempic, generic Wegovy, or generic Zepbound.
  • The offer uses a very low price but hides dose, shipping, or follow-up costs.
  • The provider cannot explain why compounding is clinically necessary for you.
  • The product label or pharmacy details cannot be verified.

Safer questions to ask

Ask whether the prescription is for an FDA-approved product or a compounded product. If compounded, ask for the active ingredient form, pharmacy name, pharmacy license, dose concentration, dosing instructions in units and milligrams, adverse-event contact path, and what happens if compounding rules change.

Why nutrition support is still needed

Compounded or brand-name, GLP-1 therapy can reduce appetite enough that people miss protein, fiber, fluids, and micronutrients. Alma focuses on that daily nutrition layer rather than medication sourcing.

How Alma Helps

Alma supports the nutrition side of GLP-1 care. It does not prescribe medication.
